Job Responsibilities

Primary responsibility for the provision of direct nursing care and services to all
program clients, developing and monitoring nursing care plans, liaison with
community health providers and physicians. Participates as a multidisciplinary team
member in treatment planning conferences with team members, families, or
community agency providers. Assumes the responsibilities of the program
director in her absence. Demonstrates knowledge of regulatory requirements
and company policies and procedures.

Job Requirements

  • Per physician’s order or as indicated by professional assessment provide direct nursing services which include but are not limited to the following:
    – storing, administering and documenting of medication
    – crisis intervention and emergency care as needed
    – performing treatments as ordered
    – transferring, ambulating, and toileting participants who need assistance
    – assessing and documenting physical and mental status of clients monthly or as indicated.
    (Includes weights, vital signs and other systemic assessment).
  • Screening and nursing assessment of potential participants as assigned
  • Coordinating the development and ongoing review of the Individual Care Plan for each program participant as assigned
  • Updating of physician’s orders, every three months or more often as needed
  • Participate with other staff in treatment planning conferences to ensure comprehensive care for program participants
  • Maintain contact with physicians, other health providers, and families to ensure comprehensive care for program participants
  • Notify caregivers of changes in participant’s clinical status
  • Ensure that updated information is received from other treatment providers as needed
  • Maintain accurate clinical records which conform to all applicable statutory and regulatory guidelines
  • Serves as Acting Director in the absence of the Program Director
  • Assist in the development and implementation of educational and inservice programs for participants, families and staff
  • Responsible for implementing nutritional services and maintain therapy services for all program participants
  • Participate in hiring, training, supervision, and evaluation of nursing department staff
  • Advocate for participant’s medical needs as indicated

Job Qualifications

• Registered Nurse, licensed to practice in the Commonwealth of Massachusetts
• Minimum of three years clinical experience in medical-surgical nursing
• Two years experience with elderly and disabled adults strongly preferred
• Supervisory and administrative experience
• CPR Certification and First Aid Training required
