Job Responsibilities

Responsible to provide personal care and support services to assist the client in the achievement of physical and emotional comfort in an adult day health care setting.

Job Requirements

  • Provides personal care services in accordance with the written plan of care, such as:
    – Obtain and records vital signs
    – Assists clients with hygiene, grooming, dressing and personal care as needed
    – Assisting with toileting and elimination and maintaining documentation of toileting schedules as required
  • Applies safety principles and proper mechanics to the performance of specific techniques of personal, supportive care, such as ambulation of clients, transferring clients, assisting with normal range of motion and position, and assists with exercise
  • Assist with breakfast, lunch and snack delivery as needed
  • Performs minor cleaning and other environmental services that facilitate receiving sanitary and safe care in the adult day health setting including daily stocking of client care items in each bathroom
  • Observes, reports and documents changes in client status or expectations of service promptly
    to a supervisor or professional
  • Understands basic elements of body functioning and reports changes in client body functions
    as indicated to appropriate personnel
  • Recognizes emergency situations and implements appropriate emergency procedures when
    indicated
  • Demonstrates utilization of good basic infection control procedures and follows company
    procedures
  • Accurately documents care provided daily and maintains flow sheets per regulatory standards
    Responsible for prevention of and reporting participant abuse, neglect, mistreatment and
    misappropriation
  • Assist with all the activities programs including arts and crafts, small group activities under
    the direction of the Director
  • Assist in the implementation of the activities goals and objectives under the supervisor’s
    direction
  • Assist with trips and other community outings outside the program
  • Provide clients individual assistance during programming as indicated
  • Recognizes the physical, emotional and developmental needs of participants, working in a
    manner that respects participants, their privacy and their property
  • Attends and participates in staff meeting and program training’s

Job Qualifications

• One or more years’ experience working with adults in a health-care or social-service setting
• Takes oral and written instruction well
• Demonstrates knowledge and skills necessary to provide care to clients
• Ability to work in an independent setting
• CPR and First Aid Certification required
