Job Responsibilities

Works under the general direction of the Program Director for non-clinical issues and under the Director of Clinical Services for clinical issues. Responsible for the orientation, supervision and evaluation of Home Care Aides, Certified Nursing Assistants and Homemaker/Companions, as applicable. Provides skilled nursing care assessments, medication management visits to clients, update clients plan of care.

1. Evaluates the nursing needs of the client; initiates, develops, implements and makes necessary revisions to the clients’ plan of care. Addresses all problems in the plan of care or documents rationale for not doing so.
2. Performs therapeutic procedures with safety, skill, efficiency and effectiveness
3. Provides supervision to all aides required by Company standards. Documents finding as required
4 Assesses Aide’s job performance and teaches techniques, clinical skills and problem solving. When necessary reports any discrepancies
to Private Care Manager
5. Participates in initial Aide orientation
6. Orients appropriate personnel to the plan of care in accordance with Company standards
7. Performs orientation or supervisory visits with Aides as directed by Program Manager
8. Completes relevant documentation with accuracy, timeliness and according to policy

Job Qualifications

– Graduate of approved diploma school of nursing, or nursing AD or BSN program
– Current Massachusetts RN license
– Two years of community health nursing, which includes orientation and supervision of Home Care Aides
– Individual must have good organizational skills, people skills and excellent clinical practice
