Job Responsibilities

As a member of the multi-disciplinary team, the LPN implements the nursing care plan developed by or in collaboration with the Community Health RN.

Job Requirements

• Maintains competency and proficiency in relation to the skills inherent and required of an LPN and as needed to safely and efficiently care for the client population served by the Agency
• Evaluates the nursing needs of the client in consultation with the primary nurse; initiates, develops, implements and makes necessary revisions to the client’s plan of care, addresses all problems in the plan of care or documents rationale for not doing so
• Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the client’s care and safety; or documents rationale for not doing so
• Communicates appropriate and necessary information to the primary nurse and/or other appropriate health care professionals as often  as needed, or upon changes in the client’s condition
• Teaches, supervises and assesses the client and family regarding information, procedures and other care needs as appropriate to the client’s condition and as ordered by M.D. or identified by the primary nurse
• Completes all documentation requirement with accuracy, timeliness and according to policy
• Understands and applies appropriate case management skills in relation to the client’s plan
of care
• Collaborate with other health care personnel to ensure that diagnosis are congruent with the
current clinical status of the client
• Positively contributes to all phases of the Quality Assessment Performance Improvement
(QAPI) program. Contributions may include; identification of problem areas; QAPI meetings or participation in performance improvement projects
• Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks
• Completes work, care and documentation with accuracy and within Agency time frames
• Requires minimal supervision and is self-directed
• Maintains professionalism throughout all aspects of the job

Job Qualifications

– Current Massachusetts LPN Nursing license
– Has at least 3 yrs nursing experience or an equivalent combination of relevant education & experience
– Home Care experience preferred
