Job Responsibilities

Access client/family social, emotional and economic needs, participates in the development of the client’s plan of care, implements counseling plan and evaluates the effectiveness of the interventions made. Is responsible to assist social workers, social worker assistants.

Job Requirements

  • Completes psychosocial assessment within 48 hours of client admission (whenever possible, makes joint admission visits with the primary nurse)
  • Participates in the development and evaluation of the Plan of Care with the physician and other members of the Interdisciplinary Team
  • Provides social work services under the direction of the client’s primary physician
  • Counsels clients/families concerning financial, social, and emotional issues as identified in the assessment
  • Maintains current knowledge of Medicare/Medicaid regulations as well as policies of private insurance companies
  • Utilizes community resources to assist in meeting client/family needs
  • Submits complete and accurate documentation of phone calls and visits according to Agency
  • Participates actively in the Interdisciplinary Team meetings
  • Assists in the Volunteer Training program
  • Supervises volunteers, completing required documentation
  • Positively contributes to all phases of the Quality Assessment Performance Improvement (QAPI) program. Contributions may include; identification of problem areas; recommendations to address problem areas; data collection; attendance at periodic QAPI meetings or participation in performance improvement projects.
  • Provides education, support and supervision of all social workers,
    social worker assistants as required or applicable.
  • Assists in orientation of new personnel
  • Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks
  • Completes work and documentation with accuracy and within Agency time frames
  • Requires minimal supervision and is self-directed

Job Qualifications

Has a Master of Social Work (MSW) degree from a school of social work accredited by the council on Social Work Education; or

Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or in psychology, sociology, or other field related to social work and is supervised by an MSW who meets all regulatory requirements

Has 1 year of social work experience in a health care setting
