Job Responsibilities

As a member of the multi-disciplinary team, the primary Hospice Nurse is responsible for coordinating all aspects of the client’s nursing care. Utilizing the nursing process (assess, plan, intervene, and evaluate), she develops the plan of care with input from the client and family, the primary physician, and all members of the hospice team. The primary nurse functions as a case manager, working collaboratively with vendors such as pharmacies and DME companies as well as insurance case managers.

Job Requirements

  • Develops an appropriate plan of care with input from the client/family, physician, and team members
  • Administers nursing care with competence and compassion
  • Displays proficiency in all aspects of professional nursing care with particular emphasis on pain management, and symptom control for the dying
  • Presents physical status of clients at Interdisciplinary Team meetings
  • Works collaboratively with all members of the hospice team and with all other concerned health care professionals
  • Keeps primary MD informed of client’s condition
  • Orients HCAs and supervises them every two weeks
  • Submits timely and accurate documentation according to Agency policies
  • Covers calls on scheduled weekends and whenever an emergency occurs
  • Participates in the Quality Improvement Program
  • Promotes the philosophy of hospice in both formal and informal settings

 

Job Qualifications

• Graduate of an approved school of nursing: diploma, Associate Degree, Baccalaureate Degree
• Current Massachusetts RN license
• Minimum of one year nursing experience, preferably three years experience, including hospice or home care nursing
• Commitment to the philosophy of hospice care
