Medical Assistance
All medical assistance care is provided according to the plan of care that is developed and updated by our professional registered nurses who perform an in-person nursing assessment for each client. Their plan of care is reviewed with each client and/or his family and customized to their wishes. Medical assistance services may include (but are not limited to) the following:
Nursing Care
Care provided by a professional registered nurse or by a trained nursing aide, following the Plan of Care developed by the RN.
Medication Management
Monitoring by a professional registered nurse to ensure that the client is following his/her doctor’s medication orders, that the client has the supply on hand they need and that the prescription reorders and refills are up to date. Filling pillboxes, serving as liaison between client, physician and pharmacy, monitoring blood pressure and other vital signs and administering medication including eye drops, B-12 shots and annual flu shots.
Medication Administration
Performed by a registered nurse, medication is given to the client, following the doctor’s orders.
Medication Reminders
Following the medication schedule as part of the Plan of Care, reminding clients when it’s time to take their medication and ensuring they comply.
Post Surgical Care
Care provided by a professional registered nurse or by a trained nursing aide, where appropriate, following the Plan of Care developed by the RN.
Nurses Aide Care and Personal Care Assistance
Following the Plan of Care developed by the professional RN, our Certified Home Health Aides and Certified Nursing Assistants provide a range of services including assistance with bathing and dressing, personal care, assistance with walking and exercises, medication reminders, respite for caregivers plus much more, as needed or requested.
Physical Therapy
Based on the evaluation and treatment plan developed by the appropriate Rehabilitation Therapist (which includes registered Physical Therapists, Occupational Therapists or Speech Therapists) a Plan of Care including treatments and exercises is determined for each client.
Occupational Therapy
Based on the evaluation and treatment plan developed by the appropriate Rehabilitation Therapist (which includes registered Physical Therapists, Occupational Therapists or Speech Therapists) a Plan of Care including treatments and exercises is determined for each client.
Speech Therapy
Based on the evaluation and treatment plan developed by the appropriate Rehabilitation Therapist (which includes registered Physical Therapists, Occupational Therapists or Speech Therapists) a Plan of Care including treatments and exercises is determined for each client.
Transportation, Accompanied by Medical Personnel
When it is deemed advisable, arrangements can be made to have a qualified medical person accompany a client to an appointment or any destination requested.
Care Management
A specially trained agency supervisor will coordinate the various care needs of the client, including scheduling and arranging transportation for medical appointments and making arrangements to resolve any non-medical needs such as locating a hair dresser who provides in the home services, assisting with finding home repair specialists and much more, upon request.
Non-Medical Assistance
Provided by specially trained aides under the guidelines of the individual’s care plan, customized care and support that helps the individual to reside more comfortably and safely at home, maximizing their ability to live as independently as possible. Services include:
Personal care including assistance with bathing, hygiene, shaving, grooming
Assistance with toileting
Assistance with dressing
Assistance with walking or exercise programs
Meal preparation
Medication reminders
Housekeeping Services
Experienced housekeepers are available for general light-housekeeping work including vacuuming, dusting, de-cluttering and straightening up, cleaning the kitchen and bathrooms, changing and making beds, laundry and ironing, meal preparation, houseplant care, removing trash, assistance with household pet care, food shopping and general errands.

Meal Preparation
Both nursing aides and housekeepers will assist clients by preparing nutritious, healthful meals including those to be eaten right away as well as meals to be reheated and eaten later such as that evening or in coming days.
Shopping and Errands
Both nursing aides and housekeepers will do food shopping and errands for their clients.
Companionship
Both nursing aides and housekeepers are appropriately trained to provide companion care for individuals who need to have someone present at home, to give family caregiver respite, or for those who would just enjoy the time and undivided attention of a “friendly visitor” to play cards, go through photo albums or just chat.
Respite to Caregivers
Companion care is available from trained staff members for those clients who need to have someone with them at home, providing their family caregiver with respite time.

Transportation
Arrangements can be made for transportation by car for medical and non-medical appointments, engagements or activities of any type.
Care for House Plants
Housekeepers are available to provide general housekeeping services which would include general care for houseplants, if requested.
Assistance with House Pets
Housekeepers are available to provide general housekeeping services which may include assisting the client with general care for small, friendly house pets, such as feeding or providing fresh water. Each request will be considered based on the client's needs and the pet’s requirements.
