Telehealth Program (telemonitoring, telemedicine)
Some patients suffering from fairly common illnesses may have a difficult time during recuperation at home if their conditions are unstable.
This could include conditions such as:
- diabetes
- hypertension pneumonia
- congestive heart failure (CHF)
- chronic obstructive pulmonary disease (COPD)
Community VNA uses the newest in technology called telemonitoring for our patients at risk due to their unstable health status. Each morning the home telemonitor unit records the patient’s vital signs and sends them to the telemonitoring nurse at Community VNA who reviews them via her computer. Even small changes can indicate the start of a problem, so the telemonitoring nurse is quick to phone the patient to find out more and solve the problem.
The telemonitor unit, which is smaller than a breadbox, is placed in the patient’s home and connected to their home telephone. There is no cost to the patient for telemonitoring and the equipment does not interfere with the telephone’s normal use. Patients and their doctors have been impressed with the positive results from home telemonitoring.
