Arranging for Services
A person may attend Norfolk Adult Day Health Center as infrequently as one day per week or as often as 5 days per week. The Center is staffed and open Monday through Friday, from 7:30AM to 4:30PM. We can assist you by arranging safe, reliable door to door, handicapped accessible transportation for those clients living in the communities of Canton, Dedham, Foxboro, Franklin, Medfield, Millis, Needham, Norfolk, Norwood, Plainville, Sharon, Walpole, Wrentham, and Westwood. Caregivers who wish to make their own transportation arrangements or drop off and pick up their loved ones themselves are not restricted to just these communities.
A Free Visit Day is available for prospective clients who are considering participating at the Center.
Each participant must have a referral from his/her doctor in order to attend the Center.

How to pay for services
Norfolk Adult Day Health Center’s services are covered by area elder service agencies, some long term care insurance plans, the Department of Mental Health, the Department of Mental Retardation, Veterans Administration, and Medicaid. We also accept private payments. Fees are kept as low as possible in order to be affordable for clients and their families.

Norfolk Adult Day Health Center is a not-for-profit health care agency and operates under a mission to enhance the community’s capacity to achieve optimal health, wellness and quality of life. Philanthropic support from individuals and organizations helps us to reach out to those in need through sliding scale and scholarship assistance whenever possible.
How to make a referral
To make a referral simply call Norfolk Adult Day Health Center between 8:00AM -4:30PM, Monday through Friday. Our medical and social services staff are also available to assist you with off-site or in-home assessments.
