Nursing Services at Community VNA

At Community VNA, our care team includes registered nurses, home care aides, social workers and rehabilitative therapists. These highly trained specialists provide our patients with a variety of clinical services. This team works hand-in-hand with the patient and family caregivers, the physician, and our other clinical staff to ensure we produce superior patient satisfaction and the highest-quality health outcomes.

Our specially trained and experienced homecare registered nurses (RNs) provide skilled nursing care which includes patient assessment, patient and caregiver education, medication administration and training, and skilled care and treatments per your doctor’s orders, including communicable disease follow-up and advanced wound care by our nurse specialists.

Rehabilitation-Physical, Occupational, Speech Therapies

Community VNA’s rehabilitation therapists help patients to safely regain their ability to move and perform their daily activities.

Our therapists have advanced training and the expertise to help patients recuperate from injury or orthopedic surgery.

Physical Therapy (PT)

Our physical therapists help you to improve your strength and balance, increasing your ability to move safely and to walk. Our PTs also help patients learn to safely use walkers, canes, wheelchairs or prostheses. We provide home safety evaluations and teach fall-prevention methods. We specialize in working with Alzheimer’s and dementia patients and their caregivers.

Occupational Therapy (OT)

Occupational therapists help patients regain strength and dexterity for the activities of daily living, such as bathing, dressing, grooming and other actions involved in self-care. Occupational therapists help those who are recuperating to build endurance, balance and stamina so they can move more easily and safely.

Speech and Language Therapy

Speech and language therapists (speech pathologists) help patients improve their swallowing abilities, regain their verbal skills or adopt new communication techniques.

Alzheimer’s Care

Community VNA provides expert care and professional services through various programs. Our staff are specially trained and experienced in providing care to those affected by Alzheimer’s disease or other forms of dementia and to helping family caregivers who are caring at home for loved ones suffering from the effects of this illness.

Community VNA utilizes advanced technology known as telehealth in order to provide the best in care for our patients. If your doctor determines that a higher level of monitoring is appropriate for your condition, Community VNA uses the newest in telehealth technology to maintain daily (or more frequent) vital sign monitoring.

Home Care Aides

At Community VNA, our Home Care Aides are specially trained and certified to provide patient assistance, including:

  • personal care - activities of daily living including bathing, dressing, toileting, etc.
  • moving safely from bed to chair or assistance with walking and climbing stairs
  • light housekeeping
  • meal preparation

Licensed Social Workers

Our licensed social workers assist patients and families with emotional challenges, advance care planning, and issues concerning health insurance and medical costs.
