Community VNA offers a wide variety of community-based health and supportive services to people of all ages that are designed to maintain and improve our clients’ wellbeing and quality of life. Nursing and Home Care Aide Services

Community VNA’s homecare registered nurses (RNs) perform patient assessment and observation, administering skilled care and treatments, as ordered by the physician, offer patient and family caregiver education, administer and explain medications, perform advanced wound care and much more.

Our certified home care aides provide personal care which includes help with bathing, dressing, moving from bed to chair, safely walking and climbing stairs; light housekeeping and meal preparation, as needed.

IV Therapy
Innovations in technology and advances in home healthcare treatments now make it possible for patients with advanced illness to safely recuperate at home. Our experienced nursing staff can utilize intravenous therapy for hydration, nutrition, antibiotics and pain management.

Medication Management
Assisting patients with their medications, including determining the proper dosages and observing any side effects, is part of the role of our registered nurses. By using telehealth monitoring , patient information is collected and analyzed daily (or more often when needed) and shared with the patient's physician to scrutinize changes and examine medication and treatment effectiveness.

Pain and Symptom Management
Our nurses are specially trained in the assessment of pain, drug and non-drug therapies, proper dosages and administration techniques. Through continuing education, our nurses remain on the cutting edge of new developments in pain management.

Case Management
Your primary nurse works with your physician, other health care specialists and your family caregivers to establish an individualized care plan that best meets your needs.

Social Services
Social workers help patients and families solve problems like insurance matters, long term care options and financial concerns with a goal of alleviating the psychological and emotional barriers that might interfere with full recovery.
